Product shall be free from defects in material and workmanship for two (2) years. Gel Pads (including Vacu-Gel™, Vacu-Form™ and FlexiForm™), Foam Positioners, OR Table Surfaces, Armboard Surfaces and Stretcher Surfaces shall be free from defects in material and workmanship for one (1) year. Warranty does not cover misuse, abuse, neglect, or lack of routine maintenance. Contact Customer Service or your Sales Representative for additional warranty information.

The guidelines below must be followed in order to initiate a return for credit:
• A Return Material Authorization (RMA) # must be obtained within thirty (30) days of invoice date by calling our Customer Service
Department (877) 458-4884.
• An RMA number is valid for fourteen (14) working days. Product received beyond this fourteen (14) day period will be refused.
• Product must be in new, unmarked, saleable condition. We will issue full credit following inspection of product. Product not in new condition
is subject to a 20% restocking fee.
• Gel Pads (including Vacu-Gel, Vacu-Form and FlexiForm), Foam Positioners, OR Table Surfaces, Armboard Surfaces and Stretcher
Surfaces returned within thirty (30) days of the invoice date are subject to a 20% restocking fee.
• Shipment damages should be reported to Customer Service Department within fourteen (14) days of invoice date.
• Custom product is not returnable.
• Disposable and Custom product is not returnable.

Allen Medical Systems' equipment (hard goods) is
serialized, and includes a serial number label. There
are two types of labels (white or silver). Both contain
six digits which may be preceded with a letter. |
|
Sterile disposable product contains a lot number on
the outside package/box. |
|
Surfaces and Pads have a letter/digit date code on the
label that is attached to the underside of the pad. |
In order for an item to be eligible for warranty, the serial number must be on the product. |
